In addition to allowing for traditional (downward) feedback collection, the collection form also provides a template for upward and self feedback collection. Meanwhile, when it comes time to aggregate and summarize the feedback, the final report template provides a simple, templated way to communicate feedback from multiple stakeholders to an employee.
