These collection form and final report templates, developed by Shane McCauley, VP of People Operations at Motive and Scale advisor, provide an off-the-shelf way to measure employees’ execution capabilities, domain expertise, user focus, collaboration skills, problem-solving skills, and direction-setting skills in any function or department.In addition to allowing for traditional (downward) feedback collection, the collection form also provides a template for upward and self feedback collection. Meanwhile, when it comes time to aggregate and summarize the feedback, the final report template provides a simple, templated way to communicate feedback from multiple stakeholders to an employee.
